  • Scan or Photo of Birth Certificate
  • Passport size picture using white background

Requirements for Registration

  • Photocopy of Birth Certificate (if not emailed)
  • Placement Slip
  • Immunization Card (to verify and return to parent) 
  • School Package (Details will be provided on Registration Day
